Have you completed a Display Screen Equipment Assessment Form?

Display Screen Equipment is:
• Any display screens where the main use of the screen is to display text, numbers and / or graphics.
• Items such as a laptop and notebook computers are subject to these Regulations.

You should complete an assessment form when:

• You commence employment
• Alter work environment or move location
• Related medical advice is received
• A member of the team is pregnant
