Student Wellbeing Referral Form

The Student Wellbeing Centre has rolled out a new referral form for all staff to use, should they have any wellbeing concerns for students.

The form asks a series of questions, and will be sent directly to the Wellbeing team, for them to assess and triage, and actioned as appropriate.

Staff can remain anonymous in the referral if they wish. It should be noted that staff do not need permission from the student to raise the concerns to the Student Wellbeing Centre.
