Student Wellbeing Referral Form

Student Wellbeing have developed a referral form for academic and professional services staff to refer students for support should there be any concerns about their wellbeing.

The form aims to gather a brief outline of the students circumstances and the reason for the referral to the wellbeing team.

Once submitted, the form will be triaged by a member of the team who will take on the concerns raised and action them as appropriate. Please note that this will be actioned during office hours, however if the matter is urgent, you can call the team on 01522 886400. If you have an urgent concern for the safety of a student outside of office hours, please call Security on 01522 886062.
